NEW PASSPORT IN PLACE OF A LOST PASSPORT
New Passport Application Approval Process
With effect from 10th August 2015, applicants applying for passports are required to provide digital photograph and fingerprints to the Department. Fingerprints will be collected from all applicants above 16 years of age. Children below 16 years of age whose passports are issued after 10th August 2015 are required to provide fingerprints to the Department upon reaching the age of 16 years
Applicants submitting passport applications through Sri Lanka Diplomatic Missions overseas could do so without providing fingerprints and digital photograph. Holders of such passports could provide fingerprints on their first arrival in Sri Lanka at the Immigration head office or any of the regional offices of the Department.

Apply a Passport through the Embassy
Duly filled following application forms should be submitted to the Consular Section of the Embassy well in advance.
Application Form K (IM 35)
All the cages from 1-17 are to be completed in Block Capital Letters. Applicant's signature should be placed in the cages given in the second page of the application in front of an authorized officer of the Embassy at the time he/she submits the application.
◾If the applicant is unable to visit the Embassy to submit the application, his/her signature and one photograph must be attested by a Justice of Peace or Attorney-at-Law/Solicitor. The attester's signature and seal should be placed on the verification photograph. The signature of the applicant should be placed well within the two cages. The attester should sign the declaration and place his seal and date. Please ensure to place the official frank of the attester below with his/her signature.
◾The passport application should be filled in English language only.

Please note that all section in this form should be completed.
◾Original Birth Certificate (Translations are not acceptable for the Passport application)
Please submit the English translation also for records, if available.
◾Four (04) photographs
Photographs should be in colour and printed in a matte or semi glossy photo quality paper. Size 3.5 cm x 4.5 cm. Photographs should be in full face view taken directly facing the camera in a plain white or off-white background, with natural facial expression and without spectacles. Both ears should be visible in the picture. Photographs should be taken within the last 03 months to reflect your current appearance.
◾Original Marriage Certificate, if the applicant has changed the name after marriage.
If the marriage has been registered outside Sri Lanka, Marriage Certificate should be countersigned by the Ministryof Foreign Affairs of the relevant country. (Eg: If the marriage has been registered in Norway, the Marriage Certificate should be countersigned by the Ministry of Foreign Affairs of Norway
◾Copy of the data page of the lost passport
◾If the applicant wishes to include the profession in the passport, original professional certificate and a letter from the employer should be submitted with the passport application. Please make sure to get the counter certification on professional certificates from Ministry of Foreign Affairs of the relevant country.
◾Original of the Police Entry/ Police Report made within 03 months to the application date, in respect of the previous/ lost passport.
◾Sri Lankan National Identity Card or the ID number, if available.
◾If the applicant or applicant's father was born outside Sri Lanka, Citizenship Certificate issued by the Department of Immigration and Emigration of Sri Lanka, should be produced.
◾Document relating to the applicant's visa status.
If the Norwegian Authorities have accepted the applicant to Norway on humanitarian grounds, a letter issued to the applicant by the Immigration Authorities or the decision of the asylum case by the relavant Norwagian Authority must be provided together with the Residance Card. Please provide such documents with their English translations.
If the applicant produces a copy of the previous passport with the application, passport fee will be NOK 2800. Otherwise the fee will be NOK 3600 and could be made in cash at the Embassy or can be transferred to the Embassy Bank Account No.7058 05 77189 DNB. Please make sure to send a copy of the deposit slip along with the application form. No refund of fees is possible in any instances.
Requirement of Departure Details.
If you apply for a passport in lieu of a lost passport, as per the regulations of the Department of Immigration and Emigration, it is necessary to provide the departure details (Number of the passport you used to leave Sri Lanka, mode of travel and date).
Issuence of Temporary Travel Documents - NMRP
NMRP will be issued to the Passport applicants who are unable to submit the previous passport details or/ and original birth certificate to prove the Sri Lankan citizenship. Subject to the approval of the Department of Immigration and Emigration of Sri Lanka.
NMRP will be valid for 06 months and can be used only for the purpose of travel to Sri Lanka.
